First, our Senior Transition Specialists will to meet you at a location of your choosing, and spend time with you having a “heart 2 heart” conversation to determine your physical, emotional, social, spiritual and financial needs. Because our conversations are done with consideration, respect and honesty, we will help you with a consultation to help understand the care needed, price, location, and personal needs that bring about good results for everyone. Health, financial and lifestyle considerations are discussed confidentially during this initial assessment and it is very important that all information regarding these topics are revealed. If your needs are for retirement/senior housing options, based on information and research, together we will decide which communities to tour.

We will help you determine if Independent Living, Assisted Living, Skilled Nursing or Long Term Care is needed. There are many living-style options to choose from. We will hold your hand throughout this process and ensure that you are asking the right questions and getting the information you need to make a decision. Once your choice has been determined, we will assist you in making sure that whatever is needed for the next part of the move-in is done in a timely manner.

We have Medicaid Specialists who have years of experience in assisting with your Medicaid Application. They will continue to work with you until Medicaid has made their decision. This is a very long and tedious job, but they will make the process much less stressful for you!

We can also recommend our Realtor who specializes in working with seniors and their families to assist you in selling your home or condo. He is extremely kind, sensitive and patient with seniors and their families and will help you make that transition easier! Due to specific training to get licensed as a Senior Real Estate Specialist, he can provide extra services and answers that some other realtors may not be able to provide.

We also have Traveling Notaries who will come to you for those important documents needing to be notarized.

At Heart 2 Heart 4 Seniors, we strive to be the best resource to assist you in many choices you have to make in this transition period. That is why we can assist with obtaining Home Care Services and VA Aid and Attendance assistance for financial assistance with your assisted living or home care services that most people do not even know exists. We have resources to help you go through your belongings to help you decide what to do with them and to help with moving if needed.
